Enrollment Documentation

Submitting the initial application during the enrollment lottery or open enrollment period does not require documentation and serves as the first step in the enrollment process. 

Once a seat is offered to you and you accept, ASTEC will request additional information to complete enrollment. 

Families must provide the required documentation and forms and pay (or the school waives) all fees within the specified timeframe in order for their child to be considered fully enrolled and eligible to attend classes.

  • Proof of residence or approved legal transfer
  • Two (2) emergency contact phone numbers
  • Proof of up-to-date immunizations
  • Economically Disadvantaged Form
  • ELL Home Language Survey
  • Paid supply and activity fees, payment arrangements, or an approved fee waiver
  • Updated Medical Form, including Self-Administration of Medication form (if applicable)
  • Scholar-signed Acceptable Computer Use and Safety Policy
  • Parent/guardian-signed Code of Conduct
  • Scholar-signed Code of Conduct
  • Official school records
  • Signed School–Parent Compact
  • Transportation Information
  • Library card (English teachers will assist scholars without a card during class)
  • Chromebook Loan Agreement Form

A scholar is not considered fully enrolled and may not attend classes until all required fees are paid (or waived) and all required documentation is submitted.
